Family Discount:
A $10 discount per child is available for families registering multiple children. If you are registering more than one child from your family, please select Yes for each child's registration. Each registered child will receive a $10 discount.
Photo & Media Permission:
Hamilton Chess Club occasionally takes photographs and videos during classes, tournaments, award ceremonies, and club events. These images may be used on our website, social media pages, newsletters, and promotional materials to showcase student achievements and club activities. Please indicate whether you give permission for your child's photo/video to be used.

Emergency Medical Treatment Consent:
I understand that should a health problem arise and I or the emergency contact is unable to be reached, I consent to emergency medical treatment, which may include surgery, ambulance transportation, or other treatment as deemed necessary by medical personnel. I consent to the release of information for insurance purposes and guarantee payment in full for all required medical costs.
Parental Consent and Release of Liability:
By clicking "Yes - I give permission" and submitting this form, I certify that I am the parent or legal guardian of the above-named participant. I release Hamilton Chess Club, its coaches, volunteers, representatives, employees, and affiliated partners from liability for injuries or damages arising from participation in club activities, except in cases of willful misconduct. I understand that participation involves inherent risks, authorize my child to participate voluntarily, agree to assume those risks, consent to necessary emergency medical care if I cannot be reached, and agree to indemnify and hold Hamilton Chess Club harmless from claims, damages, costs, or expenses arising from my child's participation.
Payment Methods - Processing Fees Information :
- Zelle: Payments have a 0% processing fee.
- Venmo: Payments have a 0% processing fee.
- Credit/Debit Card using Square: Payments are subject to a 3.3% processing fee plus a $0.30 flat transaction fee. The applicable processing fee and total amount due will be displayed above before payment is completed.
